Use this page for site corrections, provider-source questions, accessibility issues or general questions about Arizona Golden Care's decision-support content.

For provider-data corrections

Include the page URL, the field you believe is wrong and a current source that supports the correction. For licensing information, an Arizona regulatory source is preferred. A provider marketing page alone may be appropriate for current provider-reported pricing or availability but not for regulatory status.

For ALTCS questions

Arizona Golden Care cannot determine individual ALTCS eligibility by email. Use AHCCCS and the member's current plan for eligibility, network and provider-participation questions. Do not send Social Security numbers, financial documents or medical records to this email address.

For assessment questions

The online assessment processes answers in your browser and does not submit them. If you have a technical problem with the tool, describe the browser/device and what happened without sending sensitive health information.

Response expectations

This site does not promise emergency or real-time response. Do not use email for urgent safety or medical situations. Use appropriate emergency or professional services when immediate help is needed.
